How do I get the Spanish Social Security S041 form to register for healthcare in another EU country?
Reviewed by Toni Faura · Last updated:
When someone who has worked and paid contributions in Spain moves to another EU country, for example Belgium, the health insurance fund in the new country usually asks for the S041 form before registering them. This document confirms the insurance periods completed in Spain so they can be taken into account in the new country, under the principle of aggregation of periods in Regulation (EC) 883/2004.
What exactly is the S041?
The S041 is not a document issued to the individual. It is an electronic form within the EU's Electronic Exchange of Social Security Information system (EESSI). It is exchanged directly between institutions: the foreign health insurer sends the request and Spain's Instituto Nacional de la Seguridad Social (INSS, National Social Security Institute) replies with the S041, confirming the insured periods in Spain. It is the electronic equivalent of the old E104 form.
That is why the worker cannot "download" it or collect it from an office. It depends on the INSS answering the foreign institution's request.
What if the S041 is delayed?
It is common for several weeks to pass without the foreign insurer receiving a reply, which prevents the healthcare registration from being completed. In that case you can:
- Submit a written request to the INSS: file a formal request with the relevant Dirección Provincial (provincial office), international relations department. It should refer to the foreign institution's request and ask for the S041 to be issued and sent as soon as possible.
- Provide provisional evidence: obtain your informe de vida laboral (Spanish Social Security employment and contributions record), which shows your registration and deregistration dates. Some insurers accept it provisionally while the S041 arrives, but check with your insurer first.
Information needed to speed up the request
To help the INSS locate the file quickly, provide:
- The name of the foreign health insurer and its reference number for the S041 request.
- The date the insurer sent the request to Spain.
- A copy of any letter or email from the insurer confirming the request was sent.
Timescales
INSS processing times do not depend on the individual or their adviser. Even so, a formal request quoting the foreign institution's reference usually makes it easier to locate the file and speeds up the sending of the form.
